 | | For selected patients, the home offers a comfortable, healing environment for recovery from illnesses. Hospital Home Health, an affiliate of the Lincoln County Health System, is a not-for-profit agency which provides patients with cost-effective healthcare alternatives to hospitalization. Hospital Home Health ensures continuity of care from the hospital to the home. Personalized care is offered, allowing
the patient’s family to retain their identity and independence through active participation in the patient’s recovery plan.

Eligibility
Home Health Care is available for patients in need of a skilled service such as nursing or therapy on an intermittent basis. Home Health Care requires a physician’s order for services and that the patient be essentially homebound.

Our Team Approach
Hospital Home Health and Hospice use a team
approach, involving qualified professionals. Nurses, physicians, home health aides, therapists, and social workers work with the patient and their family or caregiver to ensure that the patient’s
comprehensive needs are met.
Insurance Coverage
Home Health Care and Hospice coverage is
provided by most private insurance companies as well as Medicare and TennCare. The agency will gladly offer assistance in determining coverage.

Services
• Routine Home Care: Services can be provided
in the comfortable and familiar surroundings
of home.
• Respite: To give the family or caregivers a brief
period of relief, services may be provided in a facility away from home for a limited number of days.
• General Inpatient Care: A short hospital stay may be necessary at times for symptom management, such as management of a pain crisis.
Eligibility for Hospice Care
Patients with a terminal illness and a prognosis of six months or less may qualify for Hospice care. The
patient cannot be seeking aggressive or curative
treatment for their disease. The patient must be aware of their diagnosis and be in agreement with their physician and family about the choice of Hospice care. As Hospice care is routinely provided in the home, a willing and able caregiver must be available on a 24 hour basis to assist with the care of the patient. |
