Patient Information

Your Admission

Your physician, a member of the medical staff at Lincoln Medical Center, will make arrangements for you to be admitted as a patient.

When you are admitted, you will be asked to provide various statistics and medical information and sign several forms. This information is required by law and is needed for insurance purposes. If all necessary forms are not signed when you are admitted, someone from the business office will contact you. Should your insurance company require pre-authorization before you can be admitted, you are responsible for this. We will be happy to assist you in contacting your insurance company.

Personal Items and Valuables

Please be sure to take care of personal items such as dentures, eyeglasses, contact lenses, or wristwatches while in the hospital. Do not wrap these items in a tissue, handkerchief, or linen because they may be thrown away by mistake. If you should move to another room, be sure to take your personal items with you. The hospital is not responsible for any personal property or valuables that are lost.

Valuables such as jewelry and large amounts of money should be sent home. The hospital is not responsible for lost or stolen items.

Rooms

Lincoln Medical Center provides private and semi-private rooms. In times of high occupancy, your room preference may not be available when you are admitted. You may request a transfer and be moved when another room becomes available.

Federal regulations require all room assignments be made without regard to race, color, religion, national origin, age, or disability.

Medications

Your doctor will prescribe the medications you need during your hospital stay. Unless your doctor tells you otherwise, please bring all medications you are currently taking to the hospital with you for identification. Once these have been identified, medications must be held in the pharmacy and may be picked up when you are discharged, or you may send them home via a family member.
