Who are we?
Since 1983, Lincoln Medical Home Health in Fayetteville, Tennessee, has been providing quality home health care services for patients in Lincoln County and surrounding areas. The agency is licensed by the State of Tennessee, Medicare certified, and accredited by the Joint Commission.
What kind of services are provided?
Home health services are provided to homebound patients who need intermittent skilled services. Services include:
- Skilled nursing
- Home health aide services (available if skilled services are being utilized)
- Rehabilitation services including
- Speech Therapy
- Physical Therapy
- Occupational Therapy
- Medical social services
What programs are offered?
Trained nurses and therapists assist patients and caregivers with recovery from illness or surgery, and can also provide assessment and teaching to help patients manage their diseases at home and prevent hospitalizations. Care is individualized to each patient’s needs.
Some specific treatments that are offered include:
- Telehealth monitoring
- Fall – prevention and balance training
- Post – operative physical therapy
- Medication management
- Cardiac and pulmonary care
- Catheter and ostomy care
- Diabetes teaching and care management
- Infusion therapy
- Wound care
Who Can Receive Home Health Services?
- Skilled Need: Patients receiving home health services must need trained, professional, care. Care must be considered reasonable and necessary. While personal care can be provided by a home health aide, this can only occur if the patient also requires the care of a skilled provider. The agency does not offer home-maker or companion services, although sitter lists are provided to patients on request.
- Homebound: Most, though not all, insurance companies specify that a patient must be home-bound to receive services. Homebound does NOT mean that a patient must be confined to bed or a wheelchair, or even to his house. Patients can leave home for medical treatments or visits to a doctor’s office, and may make infrequent outings to church or a hair salon. However, in order to leave the home, the patient must require great assistance (human and/or a device) and leaving must pose a taxing effort to the patient. Patients who are driving are not considered homebound.
- Intermittent. Nurses and therapists visit a few times a week or less often, and as ordered by the physician to monitor the patient’s progress and intervene when necessary.
How is home health paid?
Lincoln Medical Home Health services are covered by most private insurance companies, as well as Medicare and TennCare. Benefits and eligibility are reviewed at admission to ensure accurate billing.
