Lincoln Medical Center offers a pre-consent form for parents to give to the hospital in the event of an emergency situation while the parents are away. This form provides Lincoln Medical Center with treatment consent, as well as useful medical information. Leaving this information with those responsible for your children will let you feel more secure should a child become ill or injured while you are unavailable. The completed forms should be left with those who are caring for your children in your absence, and a copy can also be kept on file in the Emergency Room.
